The Intermediate Care Facility (ICF) Transition Program
The ICF Transition Program is a way for individuals with disabilities living in an Intermediate Care Facility (ICF) to move to another place, like a group home or other community setting. The Program is voluntary. In order to be eligible for the program, the individual must be a current resident of an ICF, currently have a one year stay in the ICF, and be eligible for Medicaid.

UDDC CPR Resources
The Utah Developmental Disabilities Council has put together a list of CPR resources. The materials listed are not UDDC materials and while this information is helpful, additional training though a certified provider is recommended.

Ready and Able Employment Resources
This state-supported resource provides information about various pieces of getting and keeping employment. This resource is for both employers and job seekers alike.

Pre-employment Transition Services (Pre-ETS) For Students With Disabilities
Pre-Employment Transition Services are offered to any student with a disability. Aids students in exploring and planning for successful future employment, through targeted training.

Utah Department of Health Children with Special Health Care Needs Programs
These programs are designed to help improve the health and quality of life for children with special health care needs, and their families, through early screening and detection, data integration, care coordination, education, intervention, and life transitions.

Community Health Centers with Dental Clinics or Connections
This list provides contact information for various dental resources in every county in Utah for those who may need help with access to dental services.

United Way Utah 211 Stay Safe Stay Connected Web Resource
Stay Safe, Stay Connected is a program, in collaboration with the Utah State Board of Education, dedicated to helping Utah students who have experienced the most school disconnect since the COVID-19 pandemic began. The goal is to connect more students and their families with resources that will help them be ready for the new school year. You can learn more at

Self-Advocate Resource for making your own supported decision making plan, alternatives to guardianship, etc. Created by the Advocates as Leaders Speakers’ Network.

College Programs for Students with Disabilities
Resource for parents and students that looks at the different colleges that offer programs for people with disabilities.

Utah Transition Action Guide
This Transition Action Guide has been prepared to provide information to key participants in the process of planning by teams for the transition from school to adult life for students with disabilities

Utah Home and Community Based Setting (HCBS) Self-Assessment Report 2019
This resource explains the self-assessment process for providers participating in Home and Community Based Settings updates and includes data from participating providers.

Utah Renter Toolkit
This renter toolkit was created by the Utah Housing Coalition and it looks at becoming a better tenant and also talks about the rights they have along with resources as well.

Autism Website Resources Utah Parent Center
This website highlights resources that are available to those experiencing autism, their families, and other interested community members.

Utah Resource Guide for Employers
This guide looks at the different organizations that Voc Rehab offers and it looks at tips on how to communicate to people with disabilities and people first language

Disability Law Center Resource Website
This website looks at the different areas that the Disability Law Center in Salt Lake City Utah provides and also shares some information about the topic along with videos as well.

Utah Disability Resource Handbook
This Resource Book looks at the different disability organizations across the state of Utah and provides contact information and the purpose of each group.

DSPD Resource Website
This website looks at the annual self-advocate and family gathering meetings DSPD recently had and it also looks at community resources for people with disabilities along with Health Information Privacy.

Learning Disabilities of Utah Resource Website
This website has different websites that can help your son/daughter with testing, tutoring, support for parents, utah services and so much more.

DSPD Job Seekers and Families
This website looks at the different organizations and resources that will help people with disabilities receive a gainful employment.

Healthcare Roadmap (CMS)
The Centers for Medicare and Medicaid has created a website that has resources on a roadmap to better care for a person with a disability. Some of the resources include a roadmap to a better healthier you, an explanation of benefits and so much more to check out.

“Let’s Work” (Wisconsin)
The Wisconsin DD Council created this guide to help teachers and their staff to support and help students transition to integrated, community employment.

Disability Law Center “Alternatives to Guardianship” Page
This link will show you fact sheets about guardianship and alternatives to guardianship. It also includes website links to the national resource center for supported decision making and so much more.

Utah Parent Center Transition Parent Handbook
This handbook helps parents become involved with their son/daughter’s IEP process and it also gives tips on how the self-advocate can lead their own IEP and so much more.
